MAPP - Mobilizing for Action through Planning and Partnerships

What is MAPP?

MAPP is a community-wide strategic planning tool for improving public health.
Through MAPP, community partners and residents will come together to:

  • Prioritize public health issues
  • Identify resources for addressing them, and
  • Take action!

What are the steps involved with MAPP?

  1. Organizing and Partnership Development
    Planning, Preparation, Recruit Partners and Participants
  2. Visioning
    Create a shared vision and common set of values which provide the framework for pursuing long-range community goals.
  3. Four MAPP Assessments
    The MAPP process includes four community assessments. These assessments provide critical insight into challenges and opportunities throughout the community.

    The four assessments include:

    • The Community Themes and Strengths Assessment
      Identifies issues that interest the community, perceptions about the quality of life, and identifies community assets
    • The Local Public Health System Assessment
      Measures the capacity and performance of the local public health system – which include the many organizations and entities that contribute to and affect the health of the residents of our community
    • The Community Health Status Assessment
      Assesses data about health status, quality of life, and risk factors in the community
    • The Forces of Change Assessment
      Identifies forces that are or will be affecting the community and the local public health system
  4. Identify Strategic Issues
    Using the results of the 4 assessments, participants identify strategic issues
  5. Formulate Goals and Strategies
    Strategies and goals are formulated to address each of the strategic issues
  6. Action Cycle
    Plan for action, execute strategies, and evaluate We’ll be using a Community Balanced Scorecard to monitor our progress

When ownership of the problems and solutions includes a broad base of the community, the effort is sustainable, actions build on collective wisdom, and resources from throughout the community contribute to health improvement.
